Woods Park Care Centre was awarded a 15 Bed convalescent care program by the Ministry of Health. This program is located in our home area of Oak Woods 3rd floor. The Convalescent Care program requires a supportive environment that is appropriate and provides, quality care to clients who need time to recover strength, endurance or functioning before returning home or transfer to a rehabilitation program. The average stay for clients is from 30 to 60 days and admissions are coordinated through the Community Care Access Centre – Simcoe County.

What is Convalescent Care?
Convalescent Care (CC) is a short stay program governed under the Nursing Homes Act, Charitable Institutions Act and Homes for the Aged and Rest Homes Act funded under the Ministry of Health and Long Term Care.

The convalescent care program is geared for clients who do not require acute
(hospital) care, but are currently unable to return home because a period of convalescence is required to recover strength, endurance or functioning.


Role of the Team
The care team will work together with each individual client to complete assessments, develop goals, implement therapeutic interventions, and monitor progress to facilitate a successful convalescence.

Members of the care team usually consist of the following professionals:

RN Convalescent Care Coordinator
Registered Practical Nurse
Personal Support Worker
Physician
Physiotherapist
Occupational Therapist
Occupational Therapy/Physiotherapy Assistant
Speech Therapist
Recreation / Restorative Care Assistant


During a clients stay we encourage each individual to take an active role
in the development of their plan of care by working closely with the care team members.
At any time we encourage clients to take part in recreation and leisure programs throughout the home. They may also choose to purchase any of our wellness services such as massage therapy, hair care, foot care, tuck shop/café
at their leisure.
For your convenience we have provided each room with a television and phone that we can activate on request at a minimal charge.


Meals and snacks:
Flexible wake up and breakfast times can be scheduled into each clients convalescent program to enable them to continue with their own routines. Regular meal times are scheduled at 8am, 12 noon, 5pm and are served in our dining area. A private dining area is available if clients have guests joining then for a meal. A variety of nutritious snacks are also available throughout the day.

developed by Canadian author Moyra Jones. The Enabling Choices™
approach encompasses many of the proven principles of the Gentlecare® philosophy. The emphasis is on flexible care, meaningful activities,
client abilities, client choice and relationships. Although Gentlecare®
was first created to care for clients with dementia; Specialty Care has
adapted the principles of this approach for all clients.

Wellness – is an underlying approach and philosophy that focuses on
the whole person; a healthy balance of mind, body and spirit.
This includes the 7 domains: Emotional, Spiritual, Physical, Vocational, Environmental, Social and Intellectual. The Enabling Choices™ approach incorporates these seven dimensions of wellness into the care and programs provided.
